Health Services
Erlanger-Elsmere Schools and HealthPoint Family Care are pleased to partner and offer school based health services to your student (s) at school. School immunizations, physical exams, sport physicals, sick care, dental services and referrals to mental health are available. HealthPoint is a private, nonprofit, medical and dental practice.
Any student (or under school-aged sibling) is able to access services.
Medical and dental services are available on Tuesdays and Wednesdays from 8:00 AM through 3:00 PM.

Important Messages for All Parents/Guardians
The state of Kentucky mandates every student have a current Commonwealth of Kentucky Immunization Certificate to attend school.
A recent amendment to the Kentucky Administrative Regulation on the immunization schedules for attending school added new immunization requirements for the school year beginning on or after July 1, 2018.
Effective July 1, 2018:
- ALL students in kindergarten through twelfth grade must show proof of having received two doses of Hepatitis A vaccine to attend school
- Students aged 16 years or older, must show proof of having received two doses of Meningococcal ACWY vaccine (MenACWY) to attend school. If the first dose of MenACWY was received at age sixteen (16) years or older, the second dose is not required for school entry.
Kentucky requires each child first entering a Kentucky public school or entering Grade 6 to present a Kentucky Preventative Health Care Examination Form. This exam may be completed up to one year prior to initial entry.
Kentucky requires proof of a vision examination by an optometrist or ophthalmologist. The Vision Exam form must be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5) or six (6) year old is enrolled in public school, public preschool, or Head Start program.
Kentucky requires proof of a dental screening or examination by a dentist, dental hygienist, physician, registered nurse, nurse practitioner, or physician assistant. The Dental Form form must be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old child is enrolled in public school.
School Based Health Care
HealthPoint provides School-Based Health and Dental programs to Erlanger Elsmere School District. Students with good health tend to be better students because their health is not interrupting their schoolwork.
Meet the Team
Joanie Combs, a Family Nurse Practitioner employed by HealthPoint, provides most of the same medical services in our schools that are available in HealthPoint offices. She has nine years of primary care experience and has been part of our school based services since 2013. Joanie, a medical assistant and the school nurse work together to provide quality care to our students. Services available on-site include:
- Well-child exams, school physicals, and sports physicals
- Immunizations and Flu Shots
- Sick visits and prescriptions of routine illnesses
- Assistance in management of chronic illnesses
- Vision screenings
- Reproductive and sexual health services
- Referral to specialty and behavioral health services
Dr. Brooke Faulkner will be providing dental services to students enrolled in the program. The dental team provides a full line of services in the school including:
- Routine dental exams with x-rays and cleanings twice a year
- Fluoride and sealant treatments
- Fillings, extractions and most procedures offered in dental office
- Over-the-counter medications (i.e. Tylenol) for dental pain
If you are interested in the above services for your child, you will need to fill out a School Based Health Care packet and return to school.

Medications at School
Students who will need to take medications at school during the day are required to have the Request to Administer medications form signed by both a doctor and parent. Both the form and medication will need to be checked in by either the school nurse or trained staff. The medication must be in a current bottle from the pharmacy with the description matching the label, along with the student's name and date of birth.
If you as a parent, along with the student's physician agree that the student is responsible enough to carry their emergency medication on them during the school day, please complete that section on the Administration of Medication Form. If a situation occurs and we no longer feel comfortable with the student carrying their medication on them, we have the right to revoke the self carry policy. We strongly encourage that a supply of this medication be signed into the office as a back up should your student not be in possession of this medication if an emergency arises.
Anyone interested in playing a sport through EES will need to have a current sports physical completed by their primary care physician.
